Forms- Effective August 1, 2016 all new protocol submissions must be via myProtocol

Submission Instructions:
No PDF protocol forms will be accepted after August 1. All annual updates and procedure modifications for existing projects must be entered into myProtocol as if it were a new project. Only indicate the number of animals that will be used from that point forward (do not include the animals that have been already used). The project will go through the review process and the expiration date will be moved forward accordingly.

For existing projects (submitted before Aug 1) that only need to add personnel, you may do so by sending an email to  Provide the protocol #, name, netID, and role of the individual(s) being added or removed.  For personnel being added, include the procedures this person will perform and describe their training and experience with those procedures.

Protocol Closure Report - This form will only be used to close existing projects not in the myProtocol system. Submit it to as you have done previously. After entering a protocol in the myProtocol system, you can then use the closing report within the system to close your project.

IACUC Adverse Events Form - You will use this form only if you have an adverse event that will not require you to submit an amendment to your existing project. Submit it to as you have done previously. However, if the adverse event requires a change to your existing project this will require a procedure modification resulting in a new submission in myProtocol. At which time, you may complete the adverse event form located in the system and attach it as part of the protocol in the attachments section.
